Character name bug?

Is there a bug that causes a name in brackets to show up after a character's name even though that character's player didn't put it there? 

ie.   FancyPantz (Geraldine Smith)    where FancyPantz was not created by Geraldine Smith and Geraldine Smith (the player's rl name but fake for the purpose of this example) has no clue who FancyPantz is

• Butlers show up as Name (PlayerName) (e.g. Ratchet (Wrench)), and I've seen player's real names on their profile pages, but I'm not entirely sure what you're referring to... are the brackets showing up in chat, in the world, in updates, or somewhere else?

• the bracketed name showed up in my history when i added the person as a friend.   They did not show up in the world or in a chat etc.
