That was two gravestones from one death... located exactly on top of each other - to the pixel. The only way you'd ever spot it would be to hit enter over the gravestone.
My gut feel is that this is the cause of the "player able to see their own gravestone" bug... it's not that the gravestone is staying there when they come back from Hell, it's more that they got given an extra one when they died.
